    Care navigation

The right next step, every time

When someone reaches out, the hardest part is often knowing where to go. Chaperone navigation turns “I don’t know who to talk to” into a clear, supported path: matched to a trained Peer Coach, routed to the right resource, and continuous across every conversation.

    What navigation does

A path to support, not a list of links

Most “find help” experiences hand people a directory and wish them luck. Chaperone listens to what someone actually needs, connects them to a human first, and keeps that connection coherent, so no one navigates alone.

    Understand the need

Natural-language access lets people describe what’s going on in their own words, with no forms, no jargon, no guessing which category they fit.

    Map the right path

Guidance suggests a support path based on what fits the person and your configuration: a Peer Coach, a program, a resource, or a higher level of care.

    Keep it continuous

What someone shares once carries forward. Context follows them across sessions, messaging, programs, and hand-offs, so they never start over.

Navigation guides people toward the right support, including clinical care, crisis services, or community resources when those are the right fit. Peer support is not therapy, clinical treatment, or crisis counseling, and intelligent guidance assists routing under human supervision; it never makes a diagnosis or replaces a coach, clinician, or crisis line.
    The journey

From “I’m not sure” to a clear next step

A typical navigation path moves quickly, and a human is part of it from the start. Here’s how a single moment of reaching out becomes ongoing, supported care.

    1 · Reach out

Someone starts in plain language: “I’ve been really anxious lately.” No category to pick, no wrong door.

    2 · Get matched

They’re matched to a trained Peer Coach who fits their needs, context, and any preferences your program supports.

    3 · Find the path

Together with their coach, they’re guided to programs, tools, and resources, and to clinical or community care when that’s the fit.

    4 · Stay connected

Check-ins, messaging, and continuity keep momentum. If needs change, the path adapts without losing the thread.

    Routing & coach matching

Matched to the right Peer Coach and the right next move

Support-path routing weighs what a person shares, what your program offers, and which Peer Coaches are the best fit. Recommendations are suggestions for people and coaches to act on. The human always decides.

    Meet the Peer Coach model
    • Need-aware routing: paths reflect what someone actually described, not a rigid menu
    • Coach matching: lived experience, language, and availability factor into the fit
    • Recommendations, not decisions: guidance proposes; coaches and people choose
    • Warm hand-offs: when a different coach or resource fits better, context comes along
    • Re-routing as needs change: paths aren’t one-and-done; they adapt over time
Getting in & getting more help

Clear ways in and clear pathways up

Navigation is responsible for two of the most important moments: how someone enters support, and what happens when they need more than peer support can offer.

    Access routing

However someone arrives (a link from their employer, a member portal, a school program, a QR code), they land in the right entry point and reach support without friction.

    • Multiple entry points routed to one coherent experience
    • Eligibility and access handled quietly in the background
    • A human connection within the first few steps

    Escalation pathways

When a situation needs more than peer support, navigation makes the right step obvious and fast, connecting people to clinical care, crisis services, or supervised support per your protocols.

    • Defined escalation protocols, configured to your organization
    • Supervision and oversight present at the decision point
    • Clear routing to clinical or crisis resources, never a dead end

    Continuity

One thread across every session, message, tool, and program

Navigation isn’t a one-time triage. Context (with appropriate consent and privacy controls) carries forward so a person never has to re-explain their story. Their Peer Coach, the programs they’re in, the check-ins they’ve completed, and the resources they’ve been pointed to all stay connected as one coherent path.

    Across sessions & messaging

Conversations pick up where they left off, with no repeated intake and no lost context between touchpoints.

    Across tools & programs

Programs and assessments connect to the navigation path, so progress informs the next recommendation.

    Across hand-offs

When the path changes, the thread stays intact. Continuity isn’t broken by a new coach or resource.

    Configurable for your population

Routing logic that fits your organization and your people

No two partners route the same way. Chaperone lets you define how access works and where paths lead, by partner, by population, and by the rules your programs require.

    Partner-specific routing logic

Each partner can route to their own programs, resource networks, and preferred pathways, so a health plan, an employer, and a school district each get navigation tuned to what they offer.

    • Connect navigation to partner programs and resource directories
    • Prioritize in-network or preferred next steps
    • Reflect each partner’s escalation and crisis protocols

    Population-specific access rules

Eligibility, age-appropriate content, and access boundaries can vary by population, so members, employees, students, and community participants each get the right experience.

    • Access rules scoped to who a person is and what they’re eligible for
    • Age-appropriate paths and content for youth-serving programs
    • Resource sets matched to the population being served

Peer support is not therapy, clinical treatment, diagnosis, crisis counseling, or emergency care. Peer Coaches provide non-clinical support, practical navigation, encouragement, and connection. When someone needs a higher level of care, Chaperone helps route them to appropriate resources. If you are in immediate danger or experiencing a crisis, call 911 or call or text 988 (Suicide & Crisis Lifeline).
